Privacy policy
TALK Speech & Language Therapy ("TALK," "we") is a healthcare practice, and the information your family shares with us is protected health information under HIPAA. This page states what we collect, how we use it, when we share it, and the rights you hold over your child’s records. The information we collect
Through our intake paperwork and in the course of care, we collect your child’s name, date of birth, address, phone number, and email; insurance information you choose to provide; medical diagnoses and speech and language diagnoses; family and guardian details; and the session and treatment records our clinicians keep as part of therapy.
How we use it
We use this information for one purpose: to provide your child’s care, which includes evaluation, treatment planning, session documentation, and billing, and to comply with the legal requirements that apply to a healthcare practice, including HIPAA.
When we share it
We share your child’s information outside the practice only with a signed Authorization for Release of Information from a parent or guardian. That is how coordination with teachers, psychologists, physicians, and other members of your child’s team happens: with your permission, made explicit in writing. TALK does not sell your information.
Like every healthcare practice, we are required by law to make certain disclosures without an authorization: to public health authorities, to health oversight agencies, in legal proceedings, to law enforcement when required by law, and the mandatory reporting of suspected abuse or neglect.
Your rights over your child’s records
- · You may inspect and copy your child’s records (copying is billed at $0.25 per page).
- · You may revoke an authorization you have given, in writing, at any time.
- · You may request that records be communicated to you by alternate means or at an alternate location.
- · You may submit a correction or a written statement of disagreement to the record.
Complaints
If you believe your family’s privacy rights have been violated, tell us so we can put it right. You are also entitled to file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights.
This website
This website sets no cookies and runs no analytics or advertising trackers. The contact form composes an email in your own mail app rather than passing your child’s story through a third-party form processor; our web host keeps ordinary server logs, and that is the extent of it. Text messaging is opt-in and governed by our texting policy.
